Integrated documentation for Telecare provision

Services around the country are currently introducing a wide range of telecare services designed to enable people to continue to live safely and independently at home. To date, however, there has been no standard approach to the specification of services, the tracking of outcomes or the monitoring of service user and carer satisfaction with what has been provided.

The FACE Telecare assessment and outcome toolset is designed to address this need and has been developed in conjunction with practitioners from around the country. This first release of the toolset introduces a documentation suite that is designed to both recognise the specialist nature of telecare provision at present and to help further the integration of telecare provision with mainstream services.

Contents of the package

The tools are designed to be easy to use with a variety of local processes and referral pathways, and can support both statutory and self-funding routes. The tools are supplied in a range of formats and layouts designed to support integration with other local documentation, mobile technology and back office systems. The contents are listed below:

Core documentation:

  • Telecare Assessment (assessment of factors relevant to provision, risks that might be reduced by telecare, person’s and carer’s view, outcomes and recommendations)
  • Response Sheet (details of responders, agreed type of response, alerts, etc.)
  • Telecare Prescription (relevant details of access, home layout and environment, installation and order form)
  • Telecare Referral Form (simple generic referral form)
